If your health plan is not listed, we may still be affiliated with them. Call our office or contact your insurance company directly to verify our participation.

Insurance Billing

We participate in most major health plans. When calling for an appointment, please notify the receptionist of your health plan coverage. Please bring your insurance information to each visit. Medicare patients can be assured that assignment is always accepted. We submit claims to primary and secondary carriers as a courtesy to you.

We will bill your insurance directly and will arrange pre-authorizations as needed. Having your insurance information prior to your visit will ensure that this process will go smoothly. Many insurance companies now require us to obtain pre-authorization for certain tests. You may be referred to us for a “test only”, however, your insurance company may require us to see you first and then get authorization for the requested test. We will obtain that authorization for you and inform you of your share of the cost prior to making your appointment.


Co-Payments and Deductibles are due at the time services are rendered. You will be informed of your responsibility prior to your visit. We accept Cash or Credit Cards for New Patient Visits and Testing.

Any checks returned by the bank will be subject to a $25.00 service fee.

We understand that medical bills can be confusing. We process insurance payments in a timely manner and balance bill the patient immediately to ensure that you receive our statement about the same time as your Explanation of Benefits from your insurance company.
